Diving medical information

If you are participating in a dive or scuba course with us you will have to review this medical form. You will be sent this from SSI if you are doing a course, and it is included in the interactive forms packs below and sent to you on booking a shore or boat dive. If any of the listed conditions apply to you, you must have a doctor’s approval before you can dive. The above form includes a section for your doctor to attest to your fitness to dive.
